Donor accident insurance

Did you suffer an injury or health problem after your donation that required medical attention? First of all, we apologize for the inconvenience. You may seek reimbursement from our insurance company for your personal share of expenses in the event of an injury or accident.

How to proceed.

1. Fill out this declaration form ( Dutch–English) (Section [A], items 2, 3, 4, 5, and, if applicable, 6) andsign it (at the bottom of pages 2 and 3). The medical certificate (Section [C]) must be completed and signed by your attending physician.

2. Send the completed and signed form to:

3. We provide the form to our insurance company, who will contact you.

You do not need to send receipts to us. Please keep them until the insurance company asks you for them. With receipts of assistance you should first visit your health insurance company, which will issue you a receipt for your personal share of the cost.
