The starting product for the preparation of a washed platelet concentrate is a platelet concentrate, pathogen-reduced, either a standard platelet concentrate or a single-donor platelet concentrate.
In a single wash, the present storage medium consisting of ± 60% platelet preservative (PAS) and 40% plasma is mostly replaced by pure PAS. Washing is done manually by centrifuging the platelet concentrate, followed by pushing off the supernatant and adding fresh PAS. The entire operation is done in a closed system.
A washed platelet concentrate is used only in specific indications: the treatment of thrombopenia or bleeding in patients with antibodies to plasma proteins, especially patients with anti-IgA, and in patients who repeatedly showed a severe allergic transfusion reaction after transfusion of blood products.
Exceptionally, washing of a platelet concentrate is used in the context of transfusion of maternal platelets to a neonate with neonatal allo-immune thrombocytopenia (NAIT).

Unless where otherwise stated, the directions for use and precautions for a washed platelet concentrate are the same as for an unwashed platelet concentrate, pathogen reduced.
Since most of the plasma has been removed from the product, the hemolysin titer in the starting product should not be considered when selecting a washed single-donor platelet concentrate. Washed platelets of blood group O can therefore be administered without reservation to patients with other blood groups.
Washing a platelet concentrate cannot be done in the same thorough way as for erythrocyte concentrates because of the product and quality losses involved. Thus, not all plasma can be washed away.
Thus, the possibility of anaphylactic or severe allergic reaction remains even for a washed platelet concentrate.
The storage time for a washed platelet concentrate is 12 hours.
A platelet concentrate should not be used after the expiration date and with signs of damage or deterioration.
